Just did this up in illustator to get the measurements and lines straight. I'm off by 0.3 cm on one of the parts and had a weird extra crease mark on the chin piece. Both are easy fixes. I can't seem to get a good photo...

Still need to do some tweaking. Some feedback would be awesome.
